A patient may need to sign this form every time they are treated, or they may be able to sign one form that remains in their file for a specific amount of time.

Medical liability forms typically cover information about the patient's responsibilities regarding treatment, the liability of the hospital or medical providers, and the rights of the patient regarding privacy and other legal matters. This form must be signed and dated by either the patient themselves, or their parent, guardian, spouse, or other legal representative.
