Ub 92: What Is It?

A UB-92 form is used in the health care industry. The form is known as a Uniform or Universal Billing form. This form will be used by various hospitals, health care centers, and nursing facilities to submit a claim to Medicare or another third party health insurance company. The form will determine whether or not certain health care services provided by a large facility will be covered under the patient’s health care.

The health care facility filling out the form will need to give detailed information about the care provided. This includes surgeries, medications, hospital stays, treatments, and other expenses. The amount for each expense should be listed, as well as the grand total for the medical treatment. Both the patient and the health insurance company should receive a copy of this completed form.
