It will be used by the OPM to screen applicants for a certain employment position. It is similar to a background check and will be used to ensure that an individual is qualified for the position they are applying for.

Because this form acts as a background check, it requires a lot of detailed personal information. This will include full name, place and date of birth, previously used names, and citizenship information. After this, detailed residence information is needed for the last 5 years. You will need to include all the addresses where you have resided in the last five years. Then, you will need to do the same thing for your employers. References are needed in order to verify your employment and address history. Be sure to fill out the application completely in order to be considered for a position.
