Standard Form 50: What Is It?

A Standard Form 50 is used by the United States Office of Personnel Management. This form is known as a Notification of Personnel Action. This form is used to notify someone about actions being taken against them. This form should be kept as a record of the action, as the actions can affect an employee’s salary, position, and other qualification decisions.

Up to two actions can be listed on this one-page form. Below the actions, the name and information for the person sending the form must be listed, as well as the information for the person receiving the notice. More detailed information about the employee receiving the notice may be required, including data about their position and information about the pay and work schedules. This information should be verified by the receiving employee, and if there are any errors, they should be disputed as soon as possible.
