This form is used by the United States Small Business Administration, or SBA. The SBA Form 355 is known as an Application for Small Business Size Determination. This form will be used to determine the size of a business in order to see if they qualify for various small business benefits. For consideration, this form must be completed fully.

This form must be filled out honestly, or the business must be fined. The required information includes the business’ location, the primary function of the business, the products or services sold, and the names and contact information for the owner, directors, stockholders, and partners. The number of employees and the average amount of revenue will also be needed. This information will be verified by the Small Business Administration before your business is officially labeled as a small business.
