Form N 648: What Is It?

This form will be used by the USCIS, or United States Citizenship and Immigration Services. The Form N-648 is known as a Medical Certification for Disability Exceptions form. This form is required for those who wish to seek exemption from English and civics testing required for applicants seeking U.S. citizenship due to physical or developmental disability or mental impairment that has lasted or is expected to last over 12 months.

This form must be certified by a licensed medical professional in order for it to be submitted. This means that an in-person examination of the applicant is required. In additional to the applicant’s personal information, the medical professional will need to include their name, business address, and license information. Both parties will need to sign the completed form before it is submitted for review.
