This form will be used by the Department of the Army in the United States. Known as a Change of Address and Directory Card, this form is used for helping Army soldiers get their mail delivered to the correct address. Filling out this form is voluntary but if an Army soldier or civilian does not use this form to correct their address, they may not get their mail.
The DA form 3955 will require basic personal information such as full name and the new address. The old mailing address should also be listed. Social security number may also be needed. On the form, the party can request whether their address is shared with third parties or kept private.
